Grace Dart Extended Care Centre is looking to distinguish itself, not only by the quality of the care and services it offers, but also by the excellence it places in its relationships with members of the community.
In September 2016, Grace Dart Pavilion relocated its services from 6085 Sherbrooke St. East, to Ste. Anne’s Hospital (SAH), in Sainte-Anne-de-Bellevue. Formerly reserved for veterans, SAH was incorporated into the Montréal West Island Integrated University Health and Social Services Centre on April 1, 2016, following a transfer agreement between the Government of Canada and the Ministère de la Santé et des Services sociaux.
As a result of the closing of Grace Dart Pavilion, residents were given three relocation options: a transfer to SAH, a transfer to the Grace Dart extended care facility, or a transfer to another facility within Quebec’s health and social services network.
In all, more than 80 residents and employees of Grace Dart Pavilion and the Grace Dart extended care facility voluntarily chose to move to SAH. These new residents surely brought some of the spirit of Grace Dart with them to ensure it lives on at SAH.
